چکیده

Adrenocortical hyperplasia is uncommon in childhood. It occurs as the adrenogenital syndrome (congenital virilism in girls) which has now been shown to be due to a congenital defect in the synthesis of cortisol by the adrenal cortex. In the United States its incidence is once in every 58,000 live births (Grumbach, 1956) and it has a recessive mode of inheritance. The disease is more easily diagnosed in females than in males and this is probably the reason for the higher observed incidence in girl babies. Cushing's syndrome is a still more uncommon result of adrenocortical hyperplasia in children. Before 1949 this had not been described in a child under the age of 10 years (Wilkins, 1950) but in 1949 Chute, Robinson and Donohue described it in an 8-year-old girl. Twenty-two other cases of Cushing's syndrome under the age of 10 years have been reported and all were due to a tumour. One of the cases now to be described is a boy with Cushing's syndrome which had its onset at the age of 6 years; the other is a girl aged 21 years with congenital virilism. Both cases illustrate some problems in the pathogenesis, diagnosis and treatment of adrenocortical hyperplasia in childhood.

Adrenocortical carcinoma in a 8 years old child: a Case Report

Background:Unilateral tumors or masses of the adrenal gland are common. They are categorized as either functional (hormone-secreting) or silent and as either benign or malignant. Adrenocortical tumors are rare in childhood, with an incidence of 0.3-0.5 cases per one million child-years. Almost half of childhood tumors are adrenocortical carcinomas (ACC). Most ACCs are sporadic, but specialy i...

Cyclical Cushing syndrome presenting in infancy: an early form of primary pigmented nodular adrenocortical disease, or a new entity?

Cushing syndrome is uncommon in childhood and rare in infancy. We report the case of a 3-yr-old child who presented with symptoms of Cushing syndrome beginning shortly after birth. Her hypercortisolemia was cyclical, causing relapsing and remitting symptoms, which eventually led to suspicions of possible Munchausen syndrome by proxy. Investigation at the National Institutes of Health excluded e...

Phosphodiesterase 11A (PDE11A) and genetic predisposition to adrenocortical tumors.

PURPOSE We have reported previously nonsense inactivating mutations of the phosphodiesterase 11A (PDE11A) gene in patients with micronodular adrenocortical hyperplasia and Cushing syndrome. The aim of this study is to investigate the presence of somatic or germ-line PDE11A mutations in various types of adrenocortical tumors: ACTH-independent macronodular adrenocortical hyperplasia (AIMAH), adre...

Analysis of glucose-dependent insulinotropic peptide receptor (GIPR) and luteinizing hormone receptor (LHCGR) expression in human adrenocortical hyperplasia.

OBJECTIVE To analyze the aberrant expression of the GIPR and LHCGR in different forms of adrenocortical hyperplasia: ACTH-independent macronodular adrenal hyperplasia (AIMAH), primary pigmented nodular adrenocortical disease (PPNAD) and diffuse adrenal hyperplasia secondary to Cushing's disease (DAHCD). METHODS We quantified GIPR and LHCGR expressions using real time PCR in 20 patients with a...

Adrenocortical tumor in patients with congenital adrenal hyperplasia due to 21-hydroxylase deficiency

Adrenocortical tumour have been described in patients with 21-hydroxylase deficiency. These tumours are usually considered to be ACTH – dependent, as diffuse adrenal cortical hyperplasia is commonly seen. We report adrenal cortical tissue tumours developed in three patients with untreated congenital adrenal hyperplasia due to 21-hydroxylase deficiency. All of them had symptoms of adrenogenital ...
